Calcified canals present a challenge to anyone undertaking root canal treatment. The second mesiobuccal canal in upper molar teeth is commonly calcified and makes treating these teeth a challenge. The good news is that we have many tools available to help us locate and negotiate these canals. This talk will also describe a novel patency-filing technique to reduce the number of hand files needed to complete a case.
A smart person learns from their mistakes, but a wise person learns from the mistakes of others. In this talk, common complications that occur during the treatment process will be discussed, along with how to avoid them, how to remedy them, and how they affect the overall prognosis of treatment.
Saving natural teeth is what we must strive to do in dentistry. Often, compromised teeth require advanced restorative treatment with specialised materials. These include deep marginal elevation and techniques to manage encroaching periodontal tissues. This talk will discuss concepts and techniques we can use to preserve natural teeth and explain why implants are not always the answer.
Bioceramic materials represent a real “game changer” in our daily practice, providing patient-centered solutions from obturation to perforation repair and apical plug. The quality of the obturation has an important impact on the outcome of the treatment. The introduction of bioceramic sealers changed the scenario: thanks to their advanced properties, clinicians can now rely on sealer-based obturation techniques, shifting from warm to cold obturation methods while still achieving high-quality results. To carry out a proper obturation, it is crucial that the clinician fully understands both the techniques and the characteristics of the materials being used.
Perforation repair and apical plug procedures can be efficiently managed with bioceramic putty materials, which are ready to use and easier to apply compared to MTA, simplifying clinical procedures. This lecture aims to analyze the characteristics, benefits, and limitations of bioceramic materials, enabling clinicians to incorporate them confidently into daily practice and achieve predictable outcomes, even in cases with periapical lesions. The goal is to provide participants with detailed protocols and practical tips for achieving consistent, high-quality obturation, ultimately improving long-term, patient-centered endodontic outcomes.
Radicular separation of endodontic instruments is the worst nightmare facing every dentist in modern dentistry. Instrument separation inside the canal complicates root canal procedures and makes cleaning and shaping the canal more difficult. This hindrance can negatively affect the outcome and prognosis of the case.
The mode of separation is a complex phenomenon influenced by many factors, which I will clarify in my lecture, along with strategies for prevention.
Management of a separated instrument ranges from orthograde to surgical options. Orthograde conservative conventional approaches, including removal or bypassing of the fragment, will be the specific focus of this lecture.
A decision should be made whether to bypass or retrieve the fragment based on several influencing factors, which I will discuss.
The main goal is not only to remove the separated fragment but also to maintain the integrity of the tooth. Therefore, bypassing is often a favorable option in many situations.
I will explain when, why, and how to bypass a broken file fragment through different protocols and techniques.
Additionally, I will review all available treatment options that clinicians can perform in cases involving broken files.
Clinicians should be familiar with all available options when facing such cases, even before considering referral if needed.

Endodontic treatment encompasses a wide spectrum of clinical situations ranging from apparently straightforward cases to those of considerable complexity. However, the boundary between “simple” and “complex” is often indistinct, as anatomical variations, diagnostic uncertainties, and biological factors can significantly influence case difficulty and prognosis. This lecture aims to provide a structured framework for assessing and managing complexity in endodontic practice, integrating contemporary evidence with technological innovation.
Particular emphasis will be placed on the role of artificial intelligence (AI) in modern endodontics, focusing on diagnostic accuracy, treatment planning, and patient communication. The use of AI-based platforms, such as Diagnocat, will be illustrated through clinical examples demonstrating their potential to enhance detection of pathologies, support decision-making, and predict procedural challenges.
Specific clinical scenarios—including root resorptions, management of immature teeth, and intentional replantation—will be analyzed to highlight practical approaches and key prognostic considerations.
By combining digital tools, advanced imaging, and evidence-based protocols, clinicians can achieve greater precision, consistency, and confidence in managing both routine and complex endodontic cases. This presentation seeks to bridge the gap between innovation and clinical reality, promoting a systematic and predictable approach to endodontic complexity.

Dental practitioners are exposed to different occupational risks during the course of their professional activity, such as physical, chemical, ergonomic factors. The ergonomic risks, caused by strained posture and prolonged repetitive movements, can induce musculoskeletal disorders. That is why in my daily workflow I can’t imagine work without magnification. It helps me to take care about my musculoskeletal during 8 hours working day, 6 day per week.
Endodontics often means going beyond patterns. Each root canal case is individual, just like a molar to molar, also a canal to canal is uneven. What is crucial is not to duplicate, not to follow the routine.
Tooth anatomy is complicated, sometimes it can surprise! Premolar with three roots? For many clinicians Vertucci’s classification is the standard of root canal configuration, but everyday practice teaches to always expect the unexpected, deviations from dental anatomical norms are day-to-day reality.
Does the choice of the mechanical shaping files system affect the preparation effect? Root canal preparation techniques: reciprocation or classical rotation movements – advantages, disadvantages, possibilities. Endodontics treatment is always a challenge, it gives the opportunity to choose an unconventional approach, which supported by clinical evidence allows to achieve individual success. Greater flexibility, greater resistance to cyclic fatigue and less risk of breakage - this are the latest factors of reciprocation file system.
The clinical quality of the procedure is no longer sufficient. Complete control of intraoperative pain is now a quality standard that patients are increasingly attentive to and that we must not overlook. Endodontics is probably the dental discipline that requires the greatest precision throughout the technical procedure. The practitioner's concentration is required throughout the various clinical phases. Inadequate control of intraoperative pain can be an obstacle to this concentration.
Perfect control of intraoperative pain is necessary. However, the dental pulp, especially when inflamed, is the most sensitive tissue in the oral cavity, on which we have to intervene directly during endodontic procedures.
There are many anaesthesia techniques at our disposal. The aim of this lecture is to review the available techniques, their limitations and their benefits, depending on the planned endodontic procedures. Based on endodontic planning data, which determines the depth, duration and extent of the anaesthesia required, this lecture will look at where, what and how much to inject in order to achieve total analgesia for the comfort of both the patient and the practitioner.

The external cervical root resorptions (ECR) are destructive lesions which present in a wide variety of forms and severity, with unique challenges to the clinician with regards to its diagnosis and treatment, and that can lead to tooth loss. Its aetiology is still disputed, looking to be multifactorial. Due to the multitude of potential triggering factors, there is no available protocol on how to prevent its occurrence. From morpho-pathologic and pathophysiologic point of view, the process is assumed to be the result of the osteoclastic disruption of the unmineralized pre-cement, that would leave the underlying dentine exposed to the resorptive activity of the PDL cells. Studies proved that ECR can be found in any permanent tooth type, affecting all ages and both genders, and its distribution across the surfaces of the teeth is very arbitrary, in some cases affecting more teeth in the same patient. The variety of ECR forms makes it difficult to diagnose, misdiagnosis being easily made with other pathologies, whereas early forms are often overlooked.
The lecture will discuss how CBCT is used for diagnostic purposes, the cases in which this imaging method is recommended to be used.
The presentation will touch on several topics:
1. the limits of endodontic treatment
2. when to treat and when not to treat
3. failure of endodontic treatment, a CBCT evaluation of the causes of failure
4. perimaxillary pathologies with painful symptoms
5. differential diagnosis of odontogenic and non-odontogenic pain
We will review cases in which CBCT provided information that elucidated a complex and intricate symptomatology.
We will discuss the topic of maxillary and mandibular tumors, sinus pathology, odontogenic and non-odontogenic pain.
The presentation will be richly interspersed with specialized case studies.
